Participation Agreement

// Media Release&#13;&#10;By taking part in the activity described above, I grant Campus Outreach Columbia the full rights to use the images resulting from the photography/video filming and any reproductions or adaptations of the images for fundraising, publicity, or other purposes to help achieve Campus Outreach Columbia's organizational goals. This may include (but is not limited to), the right to use them in printed and online publicity, social media, and other marketing channels.&#13;&#10;&#13;&#10;// Release and Waiver&#13;&#10;Voluntarily and of free will, the participant (and parent/guardian if the participant is a minor) elect(s) to participate in the activity described above (the “Activity”) sponsored by the sponsoring organization described above. Furthermore, in exchange for participation in the Activity, I agree to obey all posted rules and warnings, and to comply with any given oral instructions and directions.&#13;&#10;&#13;&#10;I acknowledge that participation in the Activity involves risk to the participant (and to the participant’s parents or guardians, if the participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage.&#13;&#10;&#13;&#10;In consideration for the opportunity to participate in the Activity, the participant (or parent/guardian if the participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the Activity. The participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during the activity or during transportation to and from the activity, as well as for any medical treatment rendered to the participant that is authorized by the sponsor described above or its agents, employees, volunteers, or any other representatives (collectively referred to as the “Activity Sponsor”). Further, the participant (or parent/guardian) releases and promises to indemnify, defend, and hold harmless the Activity Sponsor for all liabilities, losses, costs, damages, claims or causes of action of any kind or nature, whatsoever, sustained by the participant as a result of or in&#13;&#10;connection with participation in the Activity, directly or indirectly, including transportation to and from the Activity, whether such injury arises out of the negligence of the Activity Sponsor, the participant, or otherwise.&#13;&#10;&#13;&#10;If a dispute over this agreement or any claim for damages arises, the participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the participant (or parent/guardian) and the Activity Sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution in accordance with the rules of the American Arbitration Association.&#13;&#10;&#13;&#10;Any legal or equitable claim that may arise from participation in the above shall be resolved under the laws of the state of South Carolina.&#13;&#10;&#13;&#10;// Medical Treatment Consent&#13;&#10;I, the participant (or the parent/guardian if the participant is a minor), understand that while every precaution will be taken to ensure the welfare and protection of the participant, the Activity Sponsor may assist in getting medical health help, and are hereby completely released from any and all liability, indemnity and restoration request, now and in the future, in the event of any accident or misfortune that may occur to the participant. In the case of an emergency, I hereby give permission for medical treatment and/or transport by the Activity Sponsor and/or First Responders to ensure proper treatment for the participant. I understand that every effort will be made to communicate to the participant (or parent/guardian) before instituting such procedures. I agree to pay, and reimburse all such doctor, ambulance, hospital, and other related fees incurred.&#13;&#10;&#13;&#10;// Cancellation & Refund Policy&#13;&#10;No cancellation or refund policy.
