Parental Consent for Independent Departure

 

 

I hereby give permission for my child/children, to leave The Lab Jiu- Jitsu School premisses on his own, without a parent, guardian, or designated adult present.

Ackowledment:

• I understand that once my child/children exits the gym premises, the staff and management of The Lab Jiu-Jitsu School are no longer responsible his safety or supervision.
• I accept full responsibility for my child's safety after they have left the gym premisses.
• This consent remains valid until I revoke it in writing.
