While we work hard to provide a safe environment, participation in physical and interactive activities comes with certain risks. Please read carefully before signing.

By signing this waiver form, I acknowledge and confirm the following:

I confirm that my child is in good health and able to participate in physical activities.I agree to inform Play n Motion staff of any allergies, medical conditions, or special requirements.I release, discharge, and hold harmless Play n Motion, its owners, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or connected with my child’s participation, whether caused by ordinary negligence or otherwise.I understand that Play n Motion is not responsible for supervision outside the designated play areas and is not liable for lost, stolen, or damaged personal property.I agree that I am financially responsible for any damages my child may cause to Play n Motion property or equipment.
I, the undersigned parent/guardian, acknowledge that my child will be participating in activities at Play n Motion that involve physical movement, including running, jumping, and interactive play. While Play n Motion takes all reasonable safety measures, I understand that risks such as slips, falls, or minor injuries may occur.
