LMHA MEMORIAL CLASSIC TEAM APPLICATION
Please Note: This form is an application for consideration to participate in the 2025 LMHA Memorial Classic. Submission does not guarantee acceptance.

📅 Tournament Dates: December 12–14, 2025

Games begin the morning of Friday, December 12. Teams travelling long distances will be scheduled as late as possible to allow for travel time.

⚔️ Divisions (EFHL Equivalent Tiers):
Tier 1, 2, 3, 4, 5, 6 & Female A (Female B teams can register as tier 4 or 5)

✅ Team Selection:
- Applications accepted until October 20, 2025.
- Teams will be notified of selection no later than November 1, 2025.
- If teams are re-tiered during their tiering rounds, their tournament placement may be adjusted accordingly.
- Tournament fee: $2,000 per team, due November 10, 2025.
- Fees are non-refundable after November 10.
- Leduc Minor Hockey Association (LMHA) and the LMHA Memorial Classic Tournament Committee are not responsible for blackout fees or reimbursement if a team is not selected to attend.

🚗Travel Permits:

Teams outside the Hockey Alberta Regional Map require a travel permit to attend. Travel permit numbers must be submitted by November 20th.

Liability Waiver: By submitting this application, the applying team, its players, coaches, managers, and affiliates agree that Leduc Minor Hockey Association (LMHA) and the LMHA Memorial Classic Tournament Committee shall not be held responsible or liable for any injury, loss, damage, or expenses incurred before, during, or after the tournament. Participation is at the sole risk of the team and its members.2. Refund Policy: The tournament fee of $2,000 per team is due by November 10, 2025 and is non-refundable after that date. LMHA is not responsible for blackout fees or reimbursement if a team is not selected to attend.3. Tournament Schedule Acknowledgment: The tournament begins on the morning of Friday, December 12, 2025. Teams may be scheduled to play during work/school hours. Teams traveling long distances will be scheduled as late as possible, but early Friday games may still be required.
