List any immediate family members by full name and relationship who are affiliated with the Dearborn County Solid Waste Management District in the capacity of employee, Executive Board member, or Advisory Committee member. (Family members that must be disclosed include parents, step-parents, grandparents, and siblings. Failure to disclose a family relationship will result in scholarship forfeiture. If you do not have any affiliations, please put N/A in the blank.
Only students who attend a high school in Dearborn County are eligible for the scholarship. The student must be a senior to apply.

Please list your involvement in extracurricular and school activities that you have participated in. We have the right to contact these people regarding your overall achievement, perseverance, cooperation, character, and work habits.
If the contact person is not affiliated with your school, please attach a document with their contact information. You do not have to include Dearborn County Recycling Center employee contact information.
