This form is for booking the APMHA Room for Pixellot Review. The room booking may ONLY be used for Pixellot Video Review and for no other purpose (team meeting/team warm-up/etc.) Bookings are first come-first serve, with a MAXIMUM of 2 sessions per month/per team.

The Ajax Pickering Minor Hockey Association Pixellot Room Bookings fall under APMHA programming and all participants are subject to the APMHA Code of Conduct. Participants who are unwilling to follow the APMHA Code of Conduct will be removed from any or all bookings.
