    I understand that training at Thee Phlebotomy Studio involves the use of needles and other medical equipment. I acknowledge that there is a risk of injury, infection, or other complications. 
     
    I voluntarily participate in this training and accept full responsibility for my actions. I release Thee Phlebotomy Studio, its owners, instructors, and staff from any claims, damages, or liability resulting from participation in this program.

    Payment & Refund Policy 
    A $150.00 seat retainer is required to secure enrollment and is non-refundable under all circumstances. 
     
    The remaining balance of $700.00 must be paid in full by the first day of class unless otherwise approved in writing.

    Attendance & Conduct Policy 
    I understand that attendance is required for successful completion of this program. Missing class time may result in additional fees or failure to complete certification requirements. 
     
    I agree to: 
    • Arrive on time and prepared 
    • Follow classroom instructions and safety protocols 
    • Maintain respectful behavior toward instructors and classmates 
     
    Failure to comply with these rules may result in removal from the program without refund.

    I understand that during my training at Thee Phlebotomy Studio, I may receive access to educational materials, class procedures, business operations, client information, and other confidential details related to the school.

    I agree not to share, copy, record, or distribute any part of the school’s curriculum, demonstrations, testing materials, or internal business information without written permission from Thee Phlebotomy Studio.

    I understand that all class materials, videos, and handouts remain the property of Thee Phlebotomy Studio and are provided only for my personal learning. Reproduction or use for personal or business gain is prohibited.

    I agree not to discuss or disclose other students’ personal information or any incidents that occur during training outside of class.

    I acknowledge that violating this agreement may result in removal from the program and potential legal action.

    By signing below, I confirm that I have read, understood, and agree to the terms of this Non-Disclosure Agreement.

    I authorize Thee Phlebotomy Studio to charge my credit or debit card for services and course fees agreed upon. I confirm that I am the authorized cardholder or have permission from the cardholder to use this card.

    I understand that deposits and fees are non-refundable unless otherwise stated in the school’s written policy. I also acknowledge that any disputes or chargebacks related to authorized charges may result in additional documentation being submitted to my bank, including this signed authorization.
