2025 - 2026 Confirmation Sponsor Form

The following questions are for you to ask your sponsor. Record what they share with you below:
How is your Catholic faith important to you?*
What do you do when you have doubts or questions about the faith?*
What motivates your faith and encourages you to continue practicing and learning?*
How has Confirmation influenced your faith life?*
Tell me about your confirmation sponsor. Why did you choose that person?*
Tell me about your confirmation saint. Why did you choose that saint?*
Do you think you would make a good sponsor? Why or why not?*
How do you think that you could be a good example and mentor in the faith for me? Would you be willing to talk about the faith with me as I prepare?*
Do you think it’s important for me to receive the sacrament of Confirmation? Why or why not?*
How will you help me to prepare to be confirmed?*
What would be your best piece of advice to me as I prepare?*
How will you continue to challenge me to grow in my faith after confirmation?*

Create three questions of your own to ask your prospective sponsor (good ones that add to your conversation, help you grow, get better advice, learn more, understand their views, etc.)

Personal Reflection Questions
After speaking with your potential sponsor, please reflect on your discussion and answer the following questions on your own.
Overall, does this person have a positive attitude about their faith? How do you know?*
How does this person show that they take their faith seriously?*
How do you know this person will challenge you to grow in your faith during class and after Confirmation?*
How does this person show they are excited about and committed to their faith and your confirmation?*
How does this person inspire you to grow/learn more about the faith? (Characteristics/actions, etc.)*
Explain any reasons you have that show how this person would not make a good sponsor: (this does not mean they can’t be your sponsor! EVERYONE is imperfect…I want to make sure you are looking at all sides of this important decision!)*
How is this person a good role model in the faith for you?*
How do you think that being your sponsor can challenge them in or better their faith?*
How do you know that you are making the best decision for you?*
What part of your discussion helped you make this decision?*
