Comfort Care Home Visit Request
  • Comfort Care Home Visit Request

  • About This Form

We know that reaching out for this kind of care can bring up a lot — questions, emotions, and uncertainty. Just getting to this point takes courage and love, and we're so grateful you're here.

This form helps us get to know your pet and understand what kind of support you're hoping for from a Comfort Care Home Visit.

Some families are looking for guidance around pain, mobility or quality of life. Others are navigating a recent diagnosis, or just want to make sure their pet is as comfortable and supported as possible in this chapter of life.

A Comfort Care Home Visit isn't just about this moment. It's about having someone to walk the road with you. We'll help you notice the changes, make sense of what's happening, and adjust the plan as your pet's needs evolve.

This is 90-minute in-home appointment. Pricing starts at $300 plus travel. Medications and any follow-up care are tailored to your pet's needs and weight and will be discussed during the visit.

Once submitted, we’ll be in touch to talk through next steps and book your appointment.

This form will take approximately 10 minutes to complete.
