  • PLAYER/PARENT and COACH WAIVER & RELEASE AGREEMENT

    In consideration of myself, spouse and child participating in any way in the New England Supreme Basketball Academy, New England Supreme, New England Supreme Athletics Inc. and Stratford basketball program related events and activities the undersigned acknowledges that the risk of injury to myself and/or child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury will always exist. Realizing that risks cannot be eliminated, I agree to the following:

    I, FOR MYSELF, SPOUSE, AND CHILD, AND TO THE GREATEST EXTENT ALLOWED BY LAW, KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS OF INJURY TO MYSELF AND/OR CHILD, both known and unknown and, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES (as defined below) or others (including fellow participants and/or players), and assume full responsibility for my own participation and/or my child's participation; and,

    I willingly agree to comply with the program's stated and customary terms and conditions for participation. If I observe any unusual significant concern for myself and/or my child's readiness for participation in the Program or in the Program itself, I will remove myself and/or child from participation and bring such to the attention of the nearest Program official immediately; and,

    I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS New England Supreme Basketball Academy, New England Supreme, New England Supreme Athletics Inc. and Stratford basketball program co-founders/owners, board members, directors, officers, officials, coaches, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the Program (referred to in this Agreement as "Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property arising out of or related to myself and/or child's involvement or participation in these Programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

    I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY AGREE TO INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities arising out of or related incident to our involvement or participation in these Programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

    I give permission for myself and/or child to be photographed or filmed for use in, including, but not limited to, publication via web site/social media, technological publications, New England Supreme Basketball Academy, New England Supreme, New England Supreme Athletics Inc. and Stratford basketball program related events and activities promotional items, basketball clinics, camps, teams, practices, leagues, tournaments, showcases and game photographs or newspapers.

    I further understand that all fees/monies are to be paid (debit/credit card, cash, Venmo or certified check only) in full prior to all services and that there is no refund for any missed clinics, sessions, camps, games, events/showcases and/or cancellations. If any event or program is cancelled by New England Supreme Basketball Academy, New England Supreme, New England Supreme Athletics Inc. and Stratford basketball program due to inclement weather, gym closure or any other reason, New England Supreme Basketball Academy, New England Supreme, New England Supreme Athletics Inc. and Stratford basketball program will try their best to make-up for their loss time. All fees are final and nonrefundable.

     

    TRAVEL TEAM EXPECTATIONS/RULES

    What we expect from the players:

    -to come prepared for practice and games with all proper attire.


    -to give full attention during basketball practices and games.


    -to always work hard to help improve basketball skills and understanding of the game.


    -to consistently demonstrate courtesy towards other players and teammates.


    -to demonstrate good sportsmanship at all times.
    -to be respectful of their coaches, referees, opponents and parents.


    -to know that the amount of playing time players will receive during games will be based on game performance, determination in practice, punctuality and attitude.


    What we expect from the parents:

    -to make sure that all players arrive to practices and games on time, and to responsibly arrange for their timely pick-up after each practice and game. (Note: all players are expected to arrive 20-25 mins prior to all games).


    -to know that parents are NOT allowed in team practices. This measure has been implemented based on past experiences where parental presence has proven to be a distraction. By limiting outside interference and maintaining a conducive environment, coaches and players can fully concentrate on skill development, teamwork, and achieving their goals.


    -to notify the coaches or team manager if any player is running late.


    -to exhibit appropriate sideline behavior and refrain from yelling instruction so that coaches can effectively provide guidance and instructions.


    -to not scold or criticize your child after a challenging game (especially on the car rides home).
    -to encourage your child and be supportive.


    -to know that there is a 24-hour rule after all games to discuss any player’s playing time. It’s essentially a cool off period that removes emotion from the conversation which allows both parties to collect their thoughts prior to speaking about the events.

     

    I have read the Travel Team Release of Liability and Assumption of Risk Agreement and the Travel Team Expectations/Rules. I fully understand its terms and acknowledge that I am giving up substantial rights by signing it. I sign this document freely and voluntarily, without any inducement. I have been advised to consult an attorney before signing this agreement.
