Haemophilia Scotland Proxy Appointment Form

Members unable to attend the Annual General Meeting may vote via a proxy appointed by them.  To do this, please complete and return this form to arrive before the date of the meeting. Your proxy does not have to be a member of Haemophilia Scotland, but should not be an employee of Haemophilia Scotland.

If you don’t know anybody who can attend the meeting as your proxy, you may nominate the Chair of the meeting, John Dearden, to act as your proxy.
