In the case of illness or injury, I authorize the ministry team or designated volunteers to seek emergency medical care from a licensed physician, hospital, or clinic for my child in the event I or other legal guardians cannot be reached.

I hereby release and hold harmless Ascension Catholic Parish, the Diocese of Wheeling-Charleston, and all associated clergy, staff, and volunteers from any and all claims, demands, or actions arising from participation in this program.
