A parent or legal guardian should sign the SEA Event Consent and Waiver only if the participant is a minor (under 18 years of age). Participants who are 18 years of age or older may self-complete and sign the form.

While submission of the digital form below is preferred, PDFs of each SEA Event Consent and Waiver (including a version in Spanish) can be downloaded from the SEA website. Once manually completed, a screenshot/image of the PDF can be uploaded using the box below.

By signing below, I certify that I am a legal adult and do consent and agree to all of the terms and conditions set forth in this Consent and Waiver. Without waving the generality of the foregoing, I specifically consent and agree for myself and my heirs, assigns, and next of kin to release and indemnify the Releasees from any and all Liability incident to my involvement as stated above, even if arising from the negligence of the Releasees, to the fullest extent permitted by law.Consent For Participation And Medical Treatment: I (the participant named above) will participate in one or more SEA events in the 2025-2026 program year (September 1, 2025 – August 31, 2026).In the event of an emergency, I grant permission and do authorize SEA coaches, staff members and volunteers to make any necessary decisions regarding my welfare and medical condition. This includes, for example, rendering preventative care or first-aid assistance or seeking treatment of medical care that seems reasonably necessary, including hospitalization, for my health and well-being.I understand and acknowledge that if any such medical treatment or care are sought including, but not limited to, the use of an ambulance for emergency transport and treatment, I am responsible for any and all fees and other expenses thereby incurred.Consent For Media Release: I authorize SEA to take photos, videos and audio recording of me in connection with my participation in SEA events. I further authorize SEA to use, without limitation and without compensation to me, my name, image and/or voice in print materials, video, online presentations, electronically or other media, for any lawful purpose such as newsletters and website content. I understand that this use of my name, image and/or voice is perpetual in time and it encompasses, without limitation, any copyright or right of publicity or privacy that I may have in my name, image and voice. This authorization, including all of its subparts, will remain effective until revoked in writing.Waiver And Release Of Liability Agreement: I acknowledge and agree that I am voluntarily participating in one or more SEA events at my own request and at my own risk. In consideration of my involvement with such event(s), I acknowledge, appreciate and agree that:I risk death and bodily injury, including paralysis, dismemberment and disability, and while particular rules of the sport, equipment, and discipline may reduce this risk, this risk of injury does exist, as well as the risk of damage to and/or loss of property; and I knowingly and freely assume all such risks, even if arising from the negligence of the Releasees (as defined below) or others; and I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual or unnecessary hazard during my presence or participation or if I observe any concern in my readiness for participation, I will immediately bring such to the attention of the nearest official and refrain from participation; andI, for myself, and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, hold harmless, and promise not to sue SEA or other sponsoring organization(s), their officers, Board of Directors, volunteers, staff, employees, sponsors and/or agents, (“Releasees”) with respect to any and all injury and loss (including death) arising from my participation, whether caused by the negligence of the Releasees or otherwise, except that which is the result of gross negligence or wanton misconduct, to the fullest extent permitted by law.
