  • Sponsor Information

A sponsor in OCIA (Order of Christian Initiation of Adults) is a practicing Catholic who accompanies a candidate or catechumen on their journey of faith. The sponsor offers spiritual support, encouragement, and guidance throughout the preparation process. They serve as a personal witness of the Catholic faith and help integrate the individual into the parish community. Sponsors are present at key liturgical moments and, for the unbaptized, may also serve as godparents at baptism.

  • Marriage and Sacramental Readiness Acknowledgment

I understand that if I am currently married, or have ever been married in the past, I must provide a copy of my Marriage License(s) and, if applicable, Divorce Decree(s). I acknowledge that some marital situations may require an Annulment or Declaration of Nullity from the Catholic Church before I can receive the Sacraments.I understand I may fully participate in OCIA and prepare for the Sacraments, but I may not be permitted to receive them until the Church has determined the validity of all current or previous marriages.I also understand that if I have already received an Annulment, Decree of Nullity, Convalidation Certificate, or Letter Granting Dispensation to marry outside the Church, I must provide a copy with this form.
  • Attendance and Commitment Acknowledgment

I understand that weekly participation is essential to the OCIA process. If a scheduling conflict arises, I will speak with the OCIA Coordinator or the Director of Religious Education as soon as possible. I am aware that makeup sessions may be available, but missing three (3) or more sessions may require a review of my readiness and commitment to continue in the program. I understand that OCIA is structured to build knowledge and faith formation progressively, and excessive absences could result in an incomplete understanding of the Catholic faith.
