  • Contact Lens Fitting Consent

    This consent explains your 60-day fitting period, follow-up visits, and prescription rules. It makes sure your lenses are safe, changes are made when needed, and your prescription keeps your eyes healthy and your vision clear.
  • What you need to know:

    • The contact lens fitting period is 60 days from your exam or training.
    • New wearers must return in 1–2 weeks for a safety check.

    If the fitting is not completed within 60 days:

    • An $80 fee will apply for additional visits or new trial lenses.
    • The doctor may change your lenses or prescription if needed for safety.

    If contact lenses are not successful:

    • Unopened, undamaged boxes (purchased within 12 months) may be refunded.
    • Exam and fitting fees are non-refundable.

    If insurance was used:

    • Stopping within 60 days will not allow new benefits for another trial.
    • If canceled, benefits are restored within 48 hours.

    Final prescription:

    • If you do not return within 60 days, the last trial lenses will become your final prescription.
    • A finalized prescription is valid for 1 year.
    • After expiration, a new exam is required before additional lenses may be ordered.
