I understand that participation in basketball training and related activities involves inherent risks including injury or illness. I voluntarily assume all risks associated with participation and release Florida Vipers Basketball Academy, Achieve University, its coaches, staff, facility owners, and affiliates from any liability arising from participation. I certify my child is physically fit to participate and understand that medical insurance is my responsibility.
Photo & Video Release Consent
I grant permission for Florida Vipers Basketball Academy and Achieve University to use photographs and videos of my child for promotional, recruiting, educational, and social media purposes without compensation.
