This agreement is entered into by and between The Planted RVT (“Service Provider”) and the undersigned Client for drop-in visits and/or dog walking services.
1. Services Provided
The Planted RVT provides professional in-home pet care services, which may include:
- Dog walking
- Potty breaks
- Feeding and fresh water
- Medication administration (as agreed upon)
- Basic cleaning of pet-related messes
- Litter box scooping
- Enrichment and companionship
- Basic health monitoring
Services will be provided as scheduled and agreed upon in advance.
2. Access to Home
Client agrees to provide safe and reliable access to the home, including:
- Working keys, lockbox code, garage code, or entry instructions
- Alarm codes and instructions if applicable
- Client is responsible for ensuring access works properly at the time of each visit.
Additional fees may apply if access is not available.
3. Pet Health & Behavior
Client certifies that:
- All pets are up to date on required vaccinations as required by Ohio law.
- Pets are free from known aggressive behavior unless disclosed in writing.
- Any medical conditions, medications, or behavioral concerns have been fully disclosed.
The Planted RVT reserves the right to refuse or terminate services for pets displaying aggression or unsafe behavior.
4. Veterinary Care & Emergencies
As a Registered Veterinary Technician, The Planted RVT will monitor pets for visible signs of illness or distress.
In the event of an emergency:
The Service Provider will attempt to contact the Client immediately.
If the Client cannot be reached, the Service Provider is authorized to seek veterinary care at the Client’s primary veterinarian or the nearest emergency facility.
Client agrees to be financially responsible for all veterinary costs incurred.
The Planted RVT is not liable for outcomes resulting from pre-existing conditions or unforeseen medical events. See veterinary treatment authorization form for more information.
5. Leashes & Safety
Dogs must have a properly fitted collar or harness with identification.
All dogs will remain leashed unless otherwise agreed upon in writing.
The Planted RVT is not responsible for loose pets if containment systems fail (fences, gates, doors).
6. Cancellations
Cancellations require at least 24 hours notice.
Late cancellations may be subject to a cancellation fee.
Holiday services may require additional notice and may include a holiday rate.
7. Payment Terms
Payment is due prior to or at time of service unless otherwise agreed.
Late payments may result in suspension of services.
Returned payments are subject to additional fees.
8. Liability
The Planted RVT agrees to provide services in a professional and responsible manner.
However, The Planted RVT is not liable for:
Injury, illness, or death due to pre-existing medical conditions
Acts of nature, theft, fire, or other unforeseen events
Escape of pets due to faulty doors, gates, fencing, or client error
Client agrees to hold The Planted RVT harmless except in cases of gross negligence or willful misconduct.
