To my knowledge, I have no medical conditions or allergy which would preclude me from having a spray tan.

I agree to waive any and all medical complications that may arise. I have been advised to discontinue use and consult a doctor if any reaction occurs.

As each pregnancy and health condition is different. You should consult a doctor prior to using. Please make sure your have permission to spray tan.

There are occasions where individuals may be allergic to one or more ingredients in a spray tan solution. Please read the list of ingredients if you have any known allergies.
The FDA has approved Dihydroxyacetone (DHA - the active ingredient in sunless spray tanning) for external application to the skin. For misting applications, the FDA recommends protective measures to prevent eye contact, inhalation or ingestion. To ensure complete protection, the FDA suggests wearing protective eyewear, nose filters, lip balm and undergarments.
There is a small percentage of people whose skin does not react favourably to spray tanning. Therefore, we do not recommend having a spray tan for the first time if you have a wedding, prom, photographic session, etc. We suggest you come one time prior to a special event.

Leather and silk may be stained by spray tanning solution, please use care.
Sunless tanning solutions do not contain any SPF protection and do not protect against the sun.

I have understood all of instructions and procedure details given to me prior the session.

I understand that I am responsible for all jewelry, clothing, accessories I wear before and after my tanning session in case of staining or damage.

While the DHA bronzing residue will appear darker after several hours, the client understands than the cosmetic bronzer will wash off during the first shower leaving a natural-looking tan.
I have been provided with spray tan care instructions, which I have read and understand completely.

While a spray tan result should last about 10 days, I understand that the results vary from one person to another according to the preparation, the maintenance, the products used on a daily basis, the life habits, the medication, the daily activities, the skin type.

I give permission to use my before and after pictures (if they are taken during appointment) for promotional purposes.

I hereby consent to receiving a spray tan. I hereby waive any and all claims or rights on my behalf as a result of any service rendered and released and forever discharge and its employees from any and all claims and responsibility or cause of action arising from the remanding of tanning services.
