Commission Request Form
A bit about the commission process and payment schedule:
Please note the current completion time for commissions is 3-4 weeks once design is approved, contract is signed and final payment has been made. Please submit the form in its entirety. Adding reference photos helps me to understand your vision and are highly recommended. Once the form is submitted I will reach out via email with any questions. Commissions start at $200, final price is dependent on design complexity and glass choice/color choice. A signed contract and $50 non-refundable deposit is due to cover design fees and supplies/glass specifically for your piece. Design work will not start until both the contract and deposit have been received. This deposit will apply to the final balance of the piece. One edit is included with the design process; I will email a copy for your review. When the design is approved, final payment is due within two weeks or the deposit will be forfeited. Once final payment is received I will begin work on your piece. Please allow 3-4 weeks to complete. I am happy to discuss if your piece is needed before that timeframe, please note a rush fee may be applied.
