1. Acknowledgment of Risk
I understand that participation in Dance Fitness and Jump Fitness classes involves physical activity that may include, but is not limited to, aerobic movement, jumping, stretching, and the use of specialized equipment such as jump boots. I acknowledge that these activities carry inherent risks of injury, including but not limited to muscle strains, sprains, falls, and equipment malfunction.

2. Health & Medical Clearance
I affirm that I am physically fit and capable of participating in this class. I have no medical condition that would prevent me from safely engaging in physical activity. I understand that it is my responsibility to consult with a physician prior to participation if I have any concerns about my health.

3. Jump Boot Rental Agreement
If I rent jump boots provided by the instructor or facility:

I agree to use the boots only as instructed and for the duration of the class.
I will inspect the boots before use and notify the instructor of any damage or concerns.
I accept full responsibility for any damage to the boots caused by misuse or negligence.
I understand that improper use may result in injury and I agree to follow all safety guidelines.
4. Release of Liability
In consideration of being allowed to participate, I hereby release and hold harmless the instructor, facility, affiliates, and any associated personnel from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained while participating in the class or using rented equipment.

5. Photo & Video Release: I grant permission for photos or videos taken during class to be used for promotional purposes.

6. Agreement & Signature
I have read and fully understand this waiver and release. I voluntarily agree to its terms and acknowledge that by signing below, I am giving up certain legal rights.
