1. Permanent Nature of Jewelry
I understand that permanent jewelry involves the welding of a jump ring or chain directly onto my body without a clasp. Permanent jewelry can be removed at any time by cutting the chain. Removal, rewelding and adjustments after your appointment may be subject to a service fee.
2. Risks Involved
I understand there are risks associated with the procedure, including but not limited to minor burns, pinching, allergic reactions, skin irritation, or jewelry snagging.
3. Aftercare Responsibility
I accept full responsibility for caring for my jewelry and monitoring for any irritation. I understand that improper care or external factors (chemicals, physical impact, etc.) may damage the piece.
4. Medical Conditions
I have disclosed any skin sensitivities, allergies to metals, or relevant medical conditions to LNKD by Jules before the procedure.
5. Release of Liability
I release and hold harmless LNKD by Jules, its owner, employees, and contractors from any and all claims, damages, or injuries resulting from this procedure.
6. Age Verification
I confirm that I am at least 18 years old, or I have parental/guardian consent if under 18. Clients under the age of 18 must have a parent or legal guardian present to sign consent forms.
