I understand that participation in dance classes, rehearsals, and performances at Naach Houston Inc. involves physical activity and carries a risk of injury. I agree to disclose any relevant medical conditions above and acknowledge that it is my responsibility to notify Naach Houston of any changes.

In the event of an emergency, I authorize Naach Houston staff to provide basic first aid and, if necessary, seek medical treatment for the student. I release Naach Houston Inc., its staff, instructors, and affiliates from any liability for injuries, medical conditions, or accidents that may occur during participation in classes or events.
