Consent & Policy Terms
By signing this form, I acknowledge and agree to the following:
• I understand there is a rare risk of allergic reaction to metals.
• I confirm that I do not have a pacemaker. (Welding should not be performed with a pacemaker; a clasp is recommended instead.)
• I understand potential risks may include, but are not limited to, allergic reactions, burns, or skin irritation.
• I agree to follow the care instructions provided by Luxe Links.
• I understand that the jewelry is intended to be permanent; however, it is not indestructible and durability may vary depending on lifestyle and wear.
• I understand that I may be required to remove my jewelry for certain medical procedures (e.g., MRI, surgery). Jewelry can be removed with scissors and re-welded at a later time.
• I grant Luxe Links permission to photograph or video my jewelry for marketing purposes.
• I understand that Luxe Links is not responsible for lost or broken chains, allergic reactions, or other damages once jewelry has been applied.
Re-Weld Policy
• Complimentary re-weld within 30 days of purchase.
• Unlimited complimentary re-welds if jewelry must be removed for medical reasons.
• After 30 days: $20 re-weld fee applies.
Refund Policy
All jewelry is custom fit and final sale. No refunds.
Photography & Video Release
I grant permission to Luxe Links and its representatives to capture and use images or videos of me and my jewelry for promotional purposes, including but not limited to social media, advertising, and marketing materials. I understand that no compensation will be provided and that Luxe Links cannot control third-party use of these materials once published.
