I hereby grant permission to St. Thomas Syro-Malabar Church Boston (SMCB) to use photographs and/or video of my child (children) taken by the Church staff or by other appropriate photographers within the church setting. I understand that these photos and / or videos may be displayed not only within the Church setting, but may also be used in publications, news releases, online, and in other communications related to the mission of St. Thomas Syro-Malabar Church Boston (SMCB). Diocesan Safe Environment Program - Parent Consent/ Decline Form

Each year, as required by St. Thomas Syro-Malabar Catholic Diocese of Chicago, instruction is provided for each grade level to assist children in recognizing and responding appropriately / assertively to improper (sexual) touch. The book or video used is age appropriate and helps the students understand they can say “no” and should tell a trusted adult about any incident. The instruction is gentle, and your child will not be exposed to specific sexual language or depictions of graphic situations or to anatomically correct language regarding private body parts. Part of the lesson explains to children that it is important that we all learn the “stop, drop, and roll’ rule even though most of us will go through life never experiencing a fire. The instruction helps children learn about these unpleasant realities in a positive way. Parents are invited to view the educational material at the Religious Education Office. Please call (your office) (508) 532-8620 to make an appointment.

The diocese requires that the following acknowledgement to be on file for each family: I acknowledge that I have received and read the Safe Environment Parent. Guide for St. Thomas Syro-Malabar Catholic Diocese of Chicago. I have received and reviewed the Religious Education Policy Handbook.

Signing this registration form indicates that the above-named child (children) and his/her (their) parents/guardians will adhere to the policies of the CCD program. Signature also provides approval for your child’s participation in the Safe Environment Program offered by the Diocese of Chicago regarding the safety of children. Note: If you like to opt out of the Safe Environment Program, please contact compliance@smcbston.org with the request. Signature gives permission for my child (children) who attend 7th grade and above, to leave the SMCB CCD without adult supervision when he/she is released from class and that SMCB CCD is not liable for the child’s safety once the child is released from the class.
