Playhaus Village LLC Liability Waiver

THIS IS A RELEASE OF LIABILITY, READ BEFORE SIGNING.

This agreement releases Playhaus Village LLC, from all liability relating to injuries that may occur on site and/or during one of the events. By signing this agreement, I agree to hold Playhaus Village LLC entirely free from any liability, including financial responsibility for injuries, regardless of whether injuries are cause by negligence.

I also acknowledge the risks involved in this activity. These include but are not limited to injury, psychological, emotional, spiritual, and physical distress, and/or death. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

I also agree to reimburse Playhaus Village LLC for damages I inflict on the property or any material goods owned by  Playhaus Village LLC, intentionally or un-intentionally during the course of the event, at all cost deemed appropriate by Playhaus Village LLC., owners or staff. I also acknowledge that Playhaus Village LLC, staff reserves the right to refuse anyone for any reason, and at any time during the event, without refunding any portion of the cost of your tickets.

By signing below, I forfeit all right to bring a suit against Playhaus Village LLC for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.

For Parents/Guardians of a participant of minor age (under 18 This is to certify that I, as guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releases, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless Playhaus Village LLC and it’s owners, employees, representatives, and agents from any and all liability incidents to my minor child’s involvement or participation in the activity as provided above. EVEN IF ARISING FROM THE NEGLIGENCE OF PLAYHAUS VILLAGE LLC to the fullest extent permitted by law.
