Photo Release Consent Form

I hereby authorize the release of the photos indicated herein. I understand that my image may be copied, edited, published or exhibited without my knowledge. 

1. Grant of Rights

I, the undersigned (“Client”), grant STUDIO58 Performance LLC (“Photographer”) and its representatives, licensees, and assigns the irrevocable, worldwide, royalty-free right to use photographs, videos, and other media taken during my branding shoot for:

 Marketing and advertising of Photographer’s services; website, social media, print, portfolio display, or any other lawful purpose.

 2. Ownership & Usage

 I understand that Photographer retains all copyrights and ownership of the media. I am granted no ownership rights, only usage as agreed upon in writing.

3. Model/Client Release

I consent to the use of my name, likeness, and biographical information in connection with the media. I waive any right to review or approve finished media.

4. Compensation

I acknowledge that the consideration for this release is the photography session. No additional payment is required for use of the media unless otherwise agreed in writing.

5. Liability Release

I release Photographer from any claims related to defamation, privacy, or moral rights in connection with the media.

6. Governing Law

This agreement is governed by the laws of the State of Arizona. Any disputes will be resolved in the courts of Arizona.

By Signing this form, I acknowledge that I completely understand all the undertaking.
