COLORS Yoga offers mobile yoga classes at a rate of $95 per 10-week session (30-minute classes). Thanks to Holy Cross Lutheran Preschool, a portion of this fee is covered for you. Families are responsible for the remaining $55 per child, per session.

LIABILITY WAIVER: At COLORS Yoga for Kids LLC, we focus on providing a safe and supportive environment for your child to develop flexibility, strength, and mindfulness. While yoga is low-impact, risks exist. Our instructors are trained in yoga, safety, and child development. By enrolling, you acknowledge these risks and agree to release COLORS Yoga for Kids LLC from any claims related to injuries or damages. Your child must be in good health to participate, and you authorize basic first aid and emergency medical treatment if needed.

 I, the undersigned, acknowledge that participation in the yoga classes provided by COLORS Yoga for Kids LLC (hereinafter "COLORS Yoga") involves physical activity that carries inherent risks. I understand that COLORS Yoga, including its instructors, employees, agents, and affiliates will exercise reasonable care in providing these services. I agree that COLORS Yoga, its instructors, employees, agents, affiliates, and Holy Cross Lutheran Preschool are not liable for any injury, accident, or damage that may occur during or as a result of my child’s participation in these classes, whether caused by negligence or otherwise.

I hereby release, discharge, and hold harmless COLORS Yoga, its instructors, employees, agents, affiliates, and Holy Cross Lutheran Preschool from any and all claims, demands, actions, or causes of action related to any injury or damages sustained by my child during the classes.

PHOTO RELEASE AUTHORIZATION: I grant COLORS Yoga the irrevocable right and permission to photograph and/or record my child during the yoga classes. I authorize COLORS Yoga to use, publish, and distribute these photographs or recordings for promotional purposes, including on the COLORS Yoga website, social media, and other marketing materials. I understand that these images will be used solely for COLORS Yoga’s promotional purposes and will not be shared, sold, or distributed to third parties.

I waive any right to inspect or approve the finished images, recordings, or accompanying text.

ACKNOWLEDGMENT AND ACCEPTANCE: By signing below, I confirm that I have read and fully understand this Liability and Photo Release Agreement. I agree to its terms and acknowledge that it constitutes a legally binding contract.

PAYMENTS FOR EACH 10-WEEK TERM ARE DUE PRIOR TO THE TERM START DATE AND WILL COVER YOUR CHILD'S YOGA CLASSES FOR THE UPCOMING 10 WEEKS. A ONE-WEEK EXTENSION IS ALLOWED FOR LATE PAYMENTS, HOWEVER, A $5.00 LATE FEE PER CHILD WILL BE ADDED TO YOUR PAYMENT. PLEASE NOTE: IF PAYMENTS, INCLUDING LATE FEES, ARE NOT CURRENT AFTER THE EXTENSION PERIOD, YOUR CHILD MAY NOT ATTEND YOGA CLASSES. THIS INCLUDES ANY LATE FEE CHARGES.

PAYMENTS ARE NOT ADJUSTED FOR MISSED CLASSES DUE TO ILLNESS, VACATIONS, OR OTHER REASONS. A CREDIT MAY BE ISSUED IF NOTICE IS GIVEN DIRECTLY TO COLORS YOGA AT LEAST 24 HOURS PRIOR TO THE ABSENCE. DECISIONS REGARDING CLASSES DURING INCLEMENT WEATHER WILL BE MADE BY HOLY CROSS LUTHERAN PRESCHOOL, AND MAKE-UP CLASSES MAY OR MAY NOT BE OFFERED AT A LATER DATE.
