Talent After Dark Entry Form

Instructions:

1. To sign up for an audition, complete and mail the registration form to Our Lady of the Holy Rosary Catholic Church, Attn: Sherrie Anderson, 44450 Highway 429, St. Amant, LA 70774. Entries will NOT be accepted the day of the event.
2. Upload your audition video to YouTube and send the video link to sanderson@olohr.com with subject link “Audition Video” and your name. Video submissions are due October 11, 2025.
3. The $40 registration fee and audio file/special requests are due by October 11, 2025. Payments accepted via PayPal or checks made payable to La Fête des Bayous.

Requirements
If you are selected to participate in the talent show, you will have six minutes to set up, perform, and tear down during the show. By signing below, you agree to perform the same performance as your audition video. Performances MUST be suitable for ALL audiences as you will be performing on church property.

Photo Release MUST BE SIGNED BY ALL PARTICIPANTS OR THEIR PARENTS:
Our Lady of the Holy Rosary Catholic Church and/or La Fête des Bayous has my permission to use my photograph or my child’s photograph. I understand that the images may be used in print publications, online publications, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

Release of Liability PARTICIPANT’S RELEASE:
By signing this form I recognize that all entrants and guests of the talent show are participating at their own risk. I release Our Lady of the Holy Rosary Catholic Church and/or other parties involved of any liability for injuries, loss, or property damage as a result of my participation in this event.
