By signing this waiver, I agree and understand the following:
1. Acknowledgment of Activities and Assumption of Risk
I acknowledge that my participation in the Maple Grove Day Retreat involves activities including, but not limited to:
- Yoga and pranayama sessions
- Sound healing, chanting, and reiki
- Soaking in natural hot spring pools
- Hiking and exploring natural terrain
- Live music gatherings
- Communal meals and tea service
I understand that these activities carry inherent risks, including physical exertion, potential for injury, slipping on wet surfaces, and exposure to natural elements. I voluntarily assume all risks associated with my participation.
2. Medical Consent
I certify that I am in good health and have no medical conditions that would prevent my participation. I consent to receive medical treatment deemed necessary in the event of injury, accident, or illness during the retreat.
3. Liability Waiver and Release
In consideration of being permitted to participate in the Maple Grove Day Retreat, I agree to release and hold harmless Amanda Hinkhouse Yoga, its facilitators (including Amanda Hinkhouse, Debi Muir, Keirshia Baker, Sami Jean, Tim Daniels, and Jill Holden), affiliates, and venue (Maple Grove Hot Springs Retreat Center) from any and all claims, liabilities, or causes of action arising out of or related to my participation in the retreat.
4. Media Consent
I grant permission for Amanda Hinkhouse Yoga to capture photographs and videos during the retreat, which may include my likeness. I consent to the use of these images for promotional and marketing purposes, including on websites, social media, and print materials.
5. Food and Dietary Acknowledgment
I understand that snacks and meals provided at the Maple Grove Day Retreat will be vegan and gluten-free, prepared with care. While efforts will be made to accommodate dietary needs, I acknowledge it is my responsibility to inform the event organizers of any restrictions or allergies prior to the retreat. I release Amanda Hinkhouse Yoga, Maple Grove Hot Springs, and all associated individuals from liability related to allergic reactions, sensitivities, or other adverse effects that may result from consuming food or beverages provided during the event.
6. Hot Springs & Outdoor Environment Acknowledgment
I understand that soaking in hot springs and exploring natural areas carry inherent risks, including slipping, dehydration, or overexposure to heat. I acknowledge that engaging in these activities is at my own risk, and I am responsible for practicing personal safety and self-care.
7. Cancellation and Refund Policy
I acknowledge the following refund policy:
75% Refund: Available until Friday, September 19, 2025.
No Refunds: After September 19, 2025. However, tickets are transferable to another individual, which I am responsible for arranging.
8. Agreement and Signature
By signing below, I affirm that I have read, understood, and agree to the terms outlined in this Liability Waiver and Consent Form.
