    CVA on behalf of our proposed CVA Mutual requests the following data for analysis and complication of coverage details and pricing. We suggest that you have your most recent insurance documents (Schedule of Insurance/Certificate of Insurance) handy for access to the relevant details required. The information collected here will only be used to assist in the design and pricing of the proposed risk protection products and will not be disclosed to any persons not associated with this purpose.

  • Business Interruption and Deductibles

    This information may be completed by entering the details from your most recent renewal.

  • Building Details

    Please provide details for ALL buildings larger than 10sqm. A 'building' is any structure that has a roof and fixed partitioning on at least 3 sides.

  • Tips

    Wall Construction: Please use this list to find the wall constuction code that apllies to the building (to the best of your knowledge).

    Finding Latitude and Longitude Using Google Maps: App - Enter the address of your property - Find the building - Long press in the centre of the building on the map an the latitude and longitude will be shown at the top of the screen (lat = first number, long = second number following the comma). Webpage - Enter the address of your property - Find the building - Right click in the centre of the building and a pop up window will show latitude and longitude (lat = first number, long = second number following the comma).

    Occupancy Type: For most Members, this will be Occupancy Type “Temporary Lodging (Hotel) [3]”. If your primary property use is short-term hire to groups, please use this selection. If your primary property use is other than this, please select the most appropriate option for your main use and/or provide details in the Comments box.

    Fire Protection and Security: Please select all that apply. For site-wide security measures such as fencing, please note on the first building only.
