This document serves as a full waiver and release of liability. By signing below, you agree to the terms and conditions outlined herein.

1. Waiver and Release of Liability

I, the undersigned parent or legal guardian, acknowledge that my child’s participation in the Wiggle Worms Workshop involves certain inherent risks. These risks include, but are not limited to, exposure to various craft materials, minor cuts or scrapes, falls, and potential choking hazards.

I understand that my child's safety is my sole responsibility, and I agree to remain present and supervise my child at all times during the class. I will ensure my child follows all instructions and safety guidelines provided by the Host/Instructor.

In consideration of my child being permitted to participate in this class, I hereby agree to release, waive, discharge, and hold harmless Wiggle Worms Workshop, its instructors, employees, and volunteers from any and all claims, demands, or causes of action arising from any injury, damage, or loss that may occur to my child or my property during the class, even if caused by the negligence of the released parties.

2. Material and Allergen Disclosure

The craft materials used in this class are non-toxic and are selected with the safety of young children in mind. Materials may include, but are not limited to:

Washable Tempera or Acrylic Paint

Construction Paper

Crayons, Markers, or Pencils

Non-toxic Glue

Common items such as cotton balls, pom-poms, pipe cleaners, and beads

Please note that certain materials, such as homemade playdough, may contain common allergens. For example, our playdough is made with flour, which contains wheat. If your child has any allergies or medical conditions, please list them below:

3. Class Rules and Expectations

I have read and understand the following class rules:

This is a parent/legal guardian-participation class. I must remain present and actively supervise my child at all times.

I understand that the goal of this class is the process of creation, not the final product. The focus is on exploration and fun.

I will ensure that my child wears appropriate clothing that can get messy.

I will not bring my child to class if they are ill or showing symptoms of illness to prevent the spread of germs.

By signing below, I certify that I have read and understood this entire document and voluntarily agree to all of its terms.

3. Photo and Video Release

I understand that photographs and/or videos may be taken during the class for use in promoting future classes or for social media.
