To book me for an event, there will be a non-refundable cost of $150 to cover the application fee for an Oregon Temporary Facility License. This application must be sent 3 weeks before your event. Information provided in this form will help me determine the cost of services. Very large parties may require a second tattoo artist of my choosing. Prices are non-negotiable.
