I acknowledge that all my questions have been answered to my full and total satisfaction. I specifically acknowledge that I have been advised of the facts and matters set forth below, and I agree as follows:

  • I,         , have been fully informed of the risks of tattooing including but not limited to infection, scarring , difficulties in detecting melanoma, and allergic reactions to tattoo pigment, latex gloves, and antibiotics. Having been informed of the potential risks associated with getting a tattoo, I still wish to proceed with tattoo application and I assume any and all risks that may arise from tattooing.

  • BY ACCEPTING THE TERMS & CONDITIONS I AS THE CLIENT DO AGREE TO FOLLOW THE LIST OF POLICIES. I THE CLIENT AM FULLY AWARE THAT ALL OF THE FOLLOWING DEPOSITS ARE NON-REFUNDABLE, IS REQUIRED TO BOOK & CAN NOT BE DISPUTED!

    $100-$150 = $50 DEPOSIT 

    $250-$500 =$100 DEPOSIT

    $500+ = 20% DEPOSIT

    I THE CLIENT UNDERSTAND THE REMAINING BALANCE IS PAID IN CASH (OR) APPLE PAY ONLY! I THE CLIENT UNDERSTAND THERE IS A 20 MINUTE GRACE PERIOD AFTER THAT A $50.00 ADDITIONAL LATE FEE WILL BE ADDED TO MY BALANCE, AFTER 30 MINUTES LATER THAN MY SCHEDULED APPOINMENT TIME THE APPOINTMENT WILL BE CANCELLED!

    I UNDERSTAND RESHEDULED APPOINTMENTS OR CHANGE ON/OF TATTOO DESIGNS SHOULD BE MADE 24HRS IN ADVANCE, I THE CLIENT AM AWARE THERE ARE NO EXTRA GUESTS. I THE CLIENT UNDERSTAND THAT FAILURE TO FOLLOW LISTED INSTRUCTIONS CAN RESULT ON BEING BANNED FROM BOOKING & ANY DISPUTES MADE BY CLIENTS WILL RESULT IN LEGAL ACTION.
