After School Club Booking Form - September

Select all of the dates when your child(ren) will be attending After School Club this month and also if they will be requiring a tea prepared by our kitchen.
After School Club will run from 3:15pm to 6pm each day (except Fridays when it closes at 5:30pm) with attendance costing £10 per day unless the child is collected before 4pm in which case the discounted rate of £5 applies. A tea will be available for an additional cost of £2 a day and will be served around 4:30pm.

If your child(ren) is/are staying for tea, please complete the table below and then select their meal choice(s). If a child's attendance at ASC is cancelled on the day (not through illness) and tea has been previously booked, a £2 charge will still be made.
