2025 CHS Restaurant Raffle Ticket Form

We will be announcing the winner at the Avondale 5k race on Saturday, October 11th! For more information or to join the race, visit us at avondale5k.com. You do not have to be present to win.

The Avondale 5k is the single largest annual fundraiser for the Charles Webb Center, the only childcare facility in the state for children with special needs, including developmental disabilities.

You must be at least 18 years old to enter. Entries must be received by 9:00 AM on October 11, 2025 to be included in the drawing. Entries limited to the first 200 entries received and fully paid. Winners will be chosen on or before 12 PM on October 11, 2025. The prizes will be delivered to the winning entrants' at the time and place of drawing, if present, or the entrants' address or at the sole option and discretion of the contest sponsor to a recipient designated by the contest winners. Prizes are not transferable and are not redeemable for cash. Contest is not sponsored or endorsed by nor affiliated with any of the restaurants mentioned in the list of prizes.
