    This Membership Agreement ("Agreement") is entered into by and between the undersigned member ("Member") and Therapeutic Massage & Wellness ("TMW"). By enrolling in the Monthly 55-Minute Massage Membership, the Member agrees to the following terms and conditions:


    1. Membership Benefits
    Each billing cycle, the Member receives:

    One (1) 55-Minute Massage (excludes Recovery Massage)
    One (1) 20-Minute Recovery Service, choice of:

    Infrared Sauna
    Red Light Therapy
    Compression Therapy

    One (1) Free Massage Add-On per session

    10% Off retail products
    10% Off additional services

    No sign-up fees

    Option to gift the monthly massage to a friend or loved one if unable to use

    2. Billing & Term
    Membership is billed $130 monthly on the original sign-up date.
    A minimum 3-month commitment is required from the date of enrollment.
    After the initial term, membership continues on a month-to-month basis until canceled in accordance with this Agreement.

    3. Session Policy

    Sessions do not roll over. Unused sessions are forfeited at the end of each billing cycle.
    One (1) rollover session is permitted per client, to be used within 30 days of the original billing period. No additional exceptions will be made.
    Additional massage sessions may be purchased at the exclusive member rate.
    All sessions must be scheduled in advance and are subject to availability.

    4. Cancellation Policy – Membership
    Membership may be canceled any time after the initial 3-month commitment.
    Cancellations must be made through the TMW app or via phone call to TMW.
    Medical cancellations will be honored immediately upon submission of approved documentation from a licensed healthcare provider.
    Cancellations made after the billing date will be processed for the following month and will not be refunded.

    5. Cancellation Policy – Appointments
    A 24-hour notice is required to cancel or reschedule any appointment.
    Failure to provide 24-hour notice will result in a fee of up to the full session value charged to the card on file.
    No-shows and late cancellations are not eligible for rescheduling or refund.

    6. Acknowledgement
    By completing membership enrollment, the Member confirms that they have read, understood, and agreed to the terms outlined in this Agreement. The Member acknowledges that failure to comply with these terms may result in additional fees or suspension of benefits.
