Part 1 – Starting Information: Therapy Dog Application

Thank you for expressing interest in a therapy dog through Serve & Support K9 Academy. Integrating a therapy dog into your life is a significant commitment that offers profound emotional and psychological benefits. We encourage those interested to apply for our program. Please note that submitting an application does not obligate you to make a decision but allows us to schedule an interview to discuss the specifics of the program and how a therapy dog may enhance the individual's life.

Application Process

To ensure we understand the applicant's needs and circumstances, we request that the person applying for the therapy dog's training complete the application themselves, if possible. If the applicant is unable to do so, please complete the application using their own words. For applicants under 18, a legal guardian or caregiver is required to read, assist in completing, and sign the application. If assistance is needed, please contact a Serve & Support K9 Academy representative to help fill out the form or to get approval for completing it independently.

Once the application is reviewed, an initial interview will be scheduled. Serve & Support K9 Academy respects the privacy of its applicants, and all information is kept confidential within the organization.

As a condition of receiving a therapy dog through Serve & Support K9 Academy, I acknowledge that all applicants must undergo a background clearance, including a criminal history check. This clearance is essential for the safety and protection of the program staff, trainers, clients, and volunteers involved in the therapy dog program.

I understand that a telephonic copy of this release shall be as valid as the original. This release is valid for all federal, state, county, and local agencies and authorities.

By signing below, I affirm that the information provided is complete and accurate to the best of my knowledge. I understand that any falsification of this information may result in the immediate rejection of my application for the therapy dog program.
