Annual Church/Charge Conference Minutes Submission

After completing an annual church/charge conference, the next step is to submit the Minutes and Pastor's Report to the Trinity Conference. Before continuing be sure you have scanned both forms and saved them into separate PDF files. (Note: Please do not submit any other church/charge conference reports that were presented to the church. We only need the Minutes and the Pastor's Report through this form. Pastor Compensation (required) and Local Church Candidacy information (if necessary) are to be submitted via separate Jotforms. All other reports remain with the church.)
