Liability Release: I understand that participation in dance classes, rehearsals, performances, competitions, travel, and related studio activities involves inherent risks of physical injury and other damages. I voluntarily assume all risks and agree to release and hold harmless A Fine Dance Studio, LLC, its owners, directors, instructors, staff, volunteers, agents, and representatives from any and all claims, demands, suits, causes of action, or liability of any kind arising from or connected with participation in studio activities.
Health & Medical Authorization. I certify that the participant is in good health and has no condition that would make participation unsafe. In the event of illness or injury during studio activities, I authorize A Fine Dance Studio staff or volunteers to obtain emergency medical care as deemed necessary, and I accept full financial responsibility for any resulting treatment.
Photo/Video Use. I understand that photos and videos may be taken during classes, rehearsals, performances, or events, and may be used by A Fine Dance Studio for educational, promotional, and advertising purposes (including social media, website, and print).
