• Sliding-Scale Program Pre-Qualification

  • Our Sliding-Scale Program helps patients who’ve genuinely exhausted all reasonable options to afford treatment.

    This quick pre-screen (about 2–3 minutes) checks basic eligibility.
    If you qualify, you’ll be taken directly to the full Sliding-Scale Application.
    If not, we’ll share next steps and other ways to get care.

    Please answer honestly—documentation will be required.

    Note: Able-bodied patients must be working full-time (40 hours/week) unless medically disabled with documentation. Some complex cases requiring physician oversight may only qualify for a partial discount.

    But you still have several good options to continue care:

    • TennCare Medicaid – Apply for state-funded coverage that may pay for your treatment. [Apply on TennCare’s Website]
    • Self-Pay Program – Pay directly at a discounted rate: $370 every four weeks or $210 every two weeks.
    • Healthcare.gov – Explore federal health insurance options that may include addiction treatment coverage. [Visit Healthcare.gov]

    💡 If your situation changes (for example, new employment status or documentation), you may reapply for the Sliding Scale Program at any time.
