Precautions are taken for the safety of your child, but in the event of accident or sickness, Cranbrook Fellowship Baptist Church, its staff, and its volunteers are hereby released from any liability. In the event that your child requires special medication, x-rays or treatment, the parents/guardians will be notified immediately.

This information will only be seen by the staff and volunteers of Cranbrook Fellowship Baptist Church. Please contact the church office for any privacy concerns you may have.

I am the parent/guardian of the child(ren) indicated above.I have read, understood and agree with the above and hereby release and discharge all parties associated with Cranbrook Fellowship Baptist Church Youth programs from any and all claims, demands, actions, and causes of action, that I/we or my/our child(ren) incur(s).If emergency medical care is needed and I am unavailable, I authorize the supervising teacher to seek medical treatment for my child(ren).I am giving my permission to take my child(ren)'s pictures for classroom projects and post them on the church website.
