When eligibility is determined, the Qualified Professional will follow this process:
- Qualified Professional will call referral source to schedule interview/tour according to current vacancies.
- The applicant and referral source will meet with Qualified Professional, Owner of H.A.N.D.S and have an opportunity to meet and get acquainted with other residents if possible. We understand the value of relationships and encourage immediate family members to participate in this process, if it is the desire of the applicant.
- The applicant and referral source will be notified of the results in writing.
When the outcome of acceptance is determined, the Qualified Professional will follow this process:
- Qualified Professional will work with applicant and support team to arrange the official move-in date.
- Qualified Professional will arrange scheduled visits for applicant to spend time with housemates prior to move in date.
- The following documents must be received by H.A.N.D.S. prior to the date of admission or admission agreement date will need to be renegotiated o Copy of applicants FL-2 within the last 30 days of admission
- Copy of TB test and results o Copy of immunization records
- Copy of all medication prescriptions (2 weeks of medications will be required on date of admission)
- Copy of Medicaid Card, Medicare Card, Social Security Card and Birth Certificate
