Littles: FUNction First Enrollment Form

We’re so excited to have you join our 4-week kids’ program designed to help little ones build strong oral habits, calmer breathing, and healthier sleep—while having fun! Program Details: 4 sessions held on Wednesdays at 4:30 pm | October 8–29 at our Jay St. location (Activity Room) | $399 and includes a MyoMunchee for each child. Please fill out the following information to reserve your child’s spot. A more detailed intake form will be sent closer to the program start. A guardian must be present with their child during every session, for the entire duration of the program.
Program Fee + What's Included

The program fee is $399, which includes 4 sessions, a MyoMunchee for your child, and access to virtual streaming/playback if you can’t attend live. Sessions will be streamed and recorded so that if you miss a class, you’ll have access to the playback at your convenience. Payment is required to secure your spot. Please note that registration fees are non-refundable, but may be applied toward future services if needed. A detailed consent form will follow with your intake paperwork.
