MBS Performance Membership - $750/month
The Performance Membership is for one individual. Access, booking privileges, weekly coaching sessions, evaluations, and member communications apply to the primary member only. Benefits are not transferable and may not be shared. To include household access, select the Family Add-On (+$100/month), which covers up to 3 immediate household members sharing the daily 90-minute simulator time. Coaching benefits remain tied to the primary member unless additional coaching is purchased.

I understand that my Performance Membership is billed on the 1st day of each month for 12 consecutive monthly payments of $750. My first charge will occur on the next 1st, unless I elect to start immediately, in which case a prorated amount for the remainder of the current month will be charged today and monthly billing will then occur on the 1st. I understand and agree that this membership includes a 12-month minimum commitment and cannot be canceled until the completion of that period.I agree to keep a valid payment method on file and understand that failed payments may result in suspension of facility access until resolved.I understand that a Mind Body Swing coach will be assigned to me based on availability, coaching specialty, and alignment with my goals.I choose to pay $750/month via ACH and understand that MBS will send an invoice to set up automatic payments.
