Withdrawal is effective on the date that the completed form is received by the Office of Academic Affairs. A student whose enrollment is interrupted for one or more semesters must apply for readmission, unless a current and approved Leave of Absence Request is on file. Consult the Academic Calendar for the tuition refund schedule and the deadline for withdrawal from classes.
