Acknowledgment of Risks
I, the undersigned participant (or parent/legal guardian if under 18), acknowledge and fully understand that martial arts seminars, demonstrations, and related activities involve physical contact and exertion that may result in injury. I understand the risks include, but are not limited to: falls, contact with other participants, equipment-related injuries, and other unforeseen hazards.
I further acknowledge that the American Martial Arts Alliance Foundation (AMAAF), Elite Publications, the Orleans Hotel & Casino, event organizers, instructors, volunteers, and sponsors assume no responsibility for injuries, losses, or damages to person or property that may occur during or in connection with participation.
Release of Liability
In consideration of being permitted to participate in the Who’s Who in the Martial Arts Seminars, I hereby:
- Release and discharge the AMAAF, Elite Publications, the Orleans Hotel & Casino, all instructors, volunteers, sponsors, and affiliates from any and all liability, claims, or demands arising from participation, whether caused by negligence or otherwise.
- Assume full responsibility for any injuries, damages, or losses sustained during participation.
- Agree to indemnify and hold harmless the above-mentioned parties from any claims, including attorney’s fees, arising from my participation.
Medical Release
I certify that I am physically fit and able to participate. I understand it is my responsibility to consult with a physician regarding any health concerns. In case of an emergency, I consent to receive medical treatment and accept responsibility for any costs incurred.
Media Release
I grant permission to the AMAAF, Elite Publications, and event organizers to photograph, record, and use my likeness, name, voice, and/or biography in connection with promotional materials, books, videos, broadcasts, websites, or other media related to the event, without compensation.
