Acknowledgment of Risk
I, the undersigned participant (or parent/guardian of a participant under 18), acknowledge that basketball is a strenuous and physically demanding sport. Participation in the Score Hoops 3-on-3 Tournament and Big Baller Brand collaboration involves inherent risks, including but not limited to, the risk of injury, illness, or accident.
By signing this waiver, I voluntarily assume all risks associated with participation. I understand and agree that myself, my child, and/or my team are responsible for our own health, safety, and conduct during this event.
Release of Liability
I hereby release, waive, and discharge Score Hoops 3-on-3, Big Baller Brand, Triple B’s Cares Foundation, Heritage Academy Laveen, and all staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of participation in this event, including but not limited to personal injury, illness, accident, property damage, or loss, except in cases of gross negligence.
Property & Conduct
I acknowledge that none of the above parties are responsible for lost, stolen, or damaged property.
I agree that all participants, parents, coaches, and spectators must remain in good character throughout the event. Failure to do so will result in removal from the event without refund.
Media & Promotional Rights
I understand and agree that by participating in this event, Score Hoops 3-on-3 and Big Baller Brand retain the full rights to all event media, including photography, video, and live streaming. Such media may be used for future promotional, marketing, and advertising purposes across social media platforms and other outlets, without further permission or compensation.
Consent & Agreement
By signing below (or checking the digital consent box if registering online), I acknowledge that I have read, understood, and agreed to all terms of this waiver and release of liability.
