I, the undersigned, hereby grant permission to Jugo Juice (the “Company”), its representatives, employees, agents, subsidiaries, franchisees, and affiliates, to record and use video and/or audio of me (the “Content”) for the purposes of marketing, advertising, and promotional content, including but not limited to publication on social media platforms (such as Instagram, Facebook, TikTok, YouTube, etc.), websites, and internal communications.
I understand and agree that:
Usage Rights: The Content may be edited, copied, exhibited, published, or distributed, and I waive the right to inspect or approve the finished product.
Ownership: The Company retains full ownership of the Content and all rights to its use.
No Compensation: I am not entitled to any compensation or royalties now or in the future related to the use of the Content.
Revocation: This permission is irrevocable once given, and the Company is under no obligation to cease using the Content upon request.
Minors: If the person being filmed is under 18 years of age, a parent or legal guardian must sign this form.
