On behalf of Lambda Kappa Omega Sorority, Inc., I would like to thank you for your interest in joining our sisterhood. Lambda Kappa Omega was founded on December 5, 2007, with a mission of creating and maintaining a strong working relationship with members, organizations, and the community in regard to high social, academic, and moral standards. Our primary goal is to implement programs designed to help meet the needs of the respective communities.

Joining Lambda Kappa Omega involves fees. The costs and payment date should have been discussed with you during the Rush and Learn. The Fees are strictly used to help maintain the sorority's financial obligations.

The initial process to begin the membership requires submitting the following information:

1. Complete the Member Application.
2. A letter of intent expressing your membership interest. Your letter should address the topics listed on this application packet's letter of intent sheet.
3. One letter of recommendation: This can be from an initiated sorority member in good standing; or from an employer or other organization with whom you are associated, acommunity service agency indicating the hours of service.
4. Professional resume. (If available)
5. Your child abuse clearance or a copy of the receipt for your online child abuse clearance or a notarized copy of the waiver included in this package.

I understand that falsification of any information on this application or attachments will eliminate me from being considered for membership into Lambda Kappa Omega Sorority, Incorporated. By signing this form, I verify that all the information I have provided is true and correct. I understand that at any time, Lambda Kappa Omega Sorority, Incorporated can rescind any rights or privileges to an applicant based on the submission of false information or documents.

*Please Note Lambda Kappa Omega does not Discriminate. Act 153 requires that all adult volunteers, responsible for the welfare of a child or having direct contact with children, provide certifications of required
background clearances including gender.

AFFIRMATION STATEMENT ASSESSMENT CHAPTER INFORMATION

Lambda Kappa Omega Sorority, Incorporated will conduct a Child abuse check on you as part of the membership application process. Such a process requires your permission for Lambda Kappa Omega Sorority, Incorporated, to obtain and hold your clearance on file. You will be responsible for the cost associated with obtaining your check.

AFFIRMATION STATEMENT

Lambda Kappa Omega Sorority, Incorporated has a strict policy against hazing. Hazing is defined as an act or series of acts that may include but are not limited to attending unauthorized rush meetings or sessions; removing garments; eating or drinking anything given to you as a requirement for membership in Lambda Kappa Omega Sorority, Incorporated; being subjected to any form of verbal, physical or mental harassment, intimidation or disgrace; “underground hazing,” “financial hazing,” “pre-pledging” or “post-initiation pledging.” Lambda Kappa Omega Sorority, Incorporated requires that those interested in membership in the Sorority support our policy against hazing, harassment, and/or humiliation.

INSTRUCTIONS:
Please record the information below regarding your involvement in community/campus activities or programs that have occurred within the last two (2) years. All applicants must submit at least one (1) but cannot exceed three (3) ECCI forms to be considered for membership in Lambda Kappa Omega Sorority, Incorporated. Additional documentation should not be submitted and subsequently will not be reviewed. This form should be completed in its entirety and any information documented without signatures will not be accepted. If still involved in the program, write “current” for End Date. The supervisor of the program must fill out and sign the bottom of the page.

Hazing

It is against the law for, potential, or active members of Lambda Kappa Omega to conduct any activities which involve “hazing.” Violations may result in loss of registration, or to local law enforcement agencies. Action and activities which may constitute hazing include, but are not limited to, the following:

• 1. Forms of physical activity not part of an organized, voluntary athletic contest or not specifically directed toward constructive work.
•  Any activity that might reasonably bring physical harm to the individual.
• Paddling, beating, or otherwise permitting someone to hit another individual.
• Requiring one to wear any degrading or uncomfortable garments.
•  Depriving one of the opportunities for sufficient sleep (6 hours per day minimum), decent and edible meals, or access to means of maintaining body cleanliness.
•  Activities interfering with one’s academic efforts by causing exhaustion, loss of sleep, or
  reasonable study time.
•  Requiring one to consume large amounts of alcohol.
• Forcing, coercing, or permitting one to eat or drink foreign or unusual substances such as raw meat, raw eggs, salt water, onions, etc.
•  Having substances such as eggs, paint, honey, etc. thrown at, poured on, or otherwise applied to the bodies of individuals.
•  Morally degrading or humiliating games or any other activities that make an individual the object of amusement, ridicule, or intimidation.
•  Kidnaps, road trips, etc., which are conducted in a manner that endangers the health or safety of an individual
• Subjecting one to cruel or unusual psychological conditions for any reason.
•  Any requirement which compels someone to participate in any activity which is illegal,
  perverse, publicly indecent, contrary to the individual’s genuine moral and/or religious beliefs, or contrary to the rules, policies, and regulations of the state.

Refund Policy

The fees paid to Lambda Kappa Omega Sorority, Inc. for the Initiation Process are nonrefundable and cannot be refunded for the following reasons:
1. Failing National Membership test.
2. Withdrawing your acceptance for membership.
3. Natural Disaster or Emergency.
4. Failure to complete the initiation process.

Ceremony and testing fees are refundable with the return of all original organizational property. 

All funds raised in the name of Lambda Kappa Omega Sorority; Inc. will remain with the sorority after the departure of the initiating member leaving the intake process.

Chapters may require a $50 hold fee to distribute the Study Book. This fee is fully refundable upon the return of the binder or successful completion of the membership process.

Ceremony Policy

Membership into Lambda Kappa Omega Sorority, Inc. requires the Membership Ceremony. This ceremony is performed for both Spring and Fall Intake at our National Conference. You are required to attend your first National Conference and participate in the National Ceremony. For Spring Intake Members Membership into Lambda Kappa Omega is temporary, pending your national ceremony. You will be granted all information and privileges as a member of Lambda Kappa Omega. However, failure to participate in the National Ceremony will Void your membership with the Sorority.

The Exceptions to this agreement are as follows:

1. Medical Emergency (Proof Required)

2. Death of an immediate family member (Proof required)

3. Natural Disaster.

4. Graduation ceremony for yourself. (Proof required)

Any of these exceptions will be accepted with Proof. A new ceremony contract and a $100 fine will also be required.

Payment Policy

The total membership fee for Lambda Kappa Omega is $400, and it is due at the first meeting. This fee may cover various items, including but not limited to: membership in Lambda Kappa Omega, a study book, testing fees, chapter fees, shipping fees, a jacket, two shirts, certificate, badge, pin, and a conference polo.

Please note that membership fees are not retroactive. Annual dues must be paid in January for both the Spring and Fall.

If you are starting a new chapter, your fees may be discounted. You will be contacted and guided by the Expansion or Membership teams.

If I or my child participates, I understand that if I decide not to participate in this process or sign this release form, this decision will not affect eligibility to participate in the organization's activities.

I understand that I may have access to any of the works or the information provided on this form, by calling the Lambda Kappa Omega contact person included on this form. I agree that my works may be used and disclosed by the Lambda Kappa Omega as described in this form as long as Lambda Kappa Omega uses
the materials in the manner described. “in a professional manner”
