Acknowledgement of risks, injury and obligations. / acknowledge that the Martial Arts is Dangerous and the activities / am about to undertake have potential dangers and by participating in them I am exposed to certain risks. I acknowledge that / have been made aware that Martial Arts is dangerous and accept that I assume the risk of, and the responsibility of any injury, illness, death, or damage to my property resulting from my participation in any activities of Bukido karate Pty Ltd or any of its instructors. I also agree that in the event that / am injured or my property is lost or damaged, I will bring no claim, legal or otherwise against Bukido Karate Pty Ltd Its operator, or its servants and agents, in respect of that injury loss or damage. Before signing this document / have read and understood it and how it effects my legal rights.
